Transferring records from Tweed Health for Everyone
You can request the transfer or release of medical records held by our practice in the following circumstances:
You are requesting your own medical records and are aged 14 years or older.
You are a parent or legal guardian requesting records for a child aged 13 years or younger.
You are the authorised executor requesting records for a deceased patient.
You are a guardian or attorney requesting records on behalf of a patient.
You would like your records transferred to another medical centre.
What We Need to Process Your Request
To process your request, you will need to:
Complete the relevant medical records request form.
Provide signed consent and proof of identity.
Pay the applicable administration fee. An invoice will be emailed or posted to you.
Allow up to five weeks for your request to be processed.
Requests From Authorised Third Parties
Authorised third parties requesting medical records on behalf of a patient should complete the Third-Party Medical Records Request Form.
This includes:
Solicitors
Insurance companies
Government agencies
Healthcare providers
Other authorised organisations
Appropriate patient consent, authority and identification documents must be included with the request.
